Registration form

Registration is required for attendance to all scientific sessions. Complete the on-line registration form according to where you would like to receive symposium related correspondence. Please provide your name, position, organization, city and state as you want this information to appear in the list of participants and your name badge.

If you are unable to access or use the on-line form, you may complete the registration form enclosed left and return it to the symposium’s secretariat by e-mail or fax. Please, use the e-mail and fax in the contacts.

Deadlines

1 December 2007 – Pre-registration

1 March 2008 – Registration (low fee)
Attention: The registration (low fee) is extended until 30 April 2008!

1 June 2008 – Final registration
